As a participant in this programs, activities and events of Icedogs Hockey Club, the undersigned acknowledges and agrees to the following terms. DISCLAIMER Icedogs Hockey, its directors, officers, members, employees, volunteers, officials, participants, clubs, agents, sponsors, owners/operators of the facility, and representatives (the “Organization”) are not responsible for any injury, damage or loss of any kind suffered by a participant during, or as a result of, any program, tournament, game, hockey school, practice, activity or event, caused in any manner whatsoever including, but not limited to, the negligence of the organization and its participants. DESCRIPTION OF RISKS In consideration of my participation in aforementioned programs, activities and events, I hereby acknowledge that I am aware of the risks and hazards associated with or related to ice hockey. The risks and hazards of ice hockey include but are not limited to injuries from; Exerting and stretching various muscle groups Vigorous Physical Exertion, rapid movements, and quick turns and stops on the ice. Strenuous cardiovascular workouts Collision with hockey boards, goalie nets and ice Being struck by hockey sticks and pucks Physical contact with other participants, resulting in injuries to the eye, teeth, face, hand and other parts of the body, bruises, sprains, cuts, scrapes, breaks, dislocations, and spinal cord injuries which may render me permanently paralyzed Variations in ice surface Travel to and from competitive events and associated non-competitive events which are integral part of the organization’s activities. I Agree I Disagree Parent Code * As a parent or guardian of a southern IceDog player, you have a responsibility to 1. Ensure to the best of your ability that your child abides by the rules of hockey, the policies of Southern IceDogs Hockey and principals of good sportsmanshop. Ensure to the best of your ability that your child respects their teammates, coaches, team officials, referees, opposing coaches, opposing team officials, and other participants in Southern IceDog games, practices and other association activities. Ensure to the best of your ability that your child conducts themselves in a manner that minimizes the risk of injury, both physical and psychological, to them self and others which includes refraining absolutely from making any derogatory comments as to another individual’s race, ethnic origin, colour, religion, gender and/or sexual orientation. 4. Refrain from crisicisim of coaches team officials, referees, teammates, opponents, opposing coaches, opposing team officials, and other participants in IceDog games and other association activities and, when you feel that criticism is warranted, you shall offer it in a manner that is fully respectful, through proper channels and away from the hockey rink in any event, never on social media platforms. 5. Refrain from directing comments which are insulting, humiliating, intimidating, hurtful, malicious, degrading, or otherwise offensive to players, team officials, coaches,referees, and association volunteers. If you feel that criticism of any such person is warranted you shall ofer it in a manner that is fully respectful, through the proper channels and away from the hockey rink, in any event, never on social media platforms. 6. Refrain from discussing the skill levels of other team players and/or coaching staff with your child or other parents and absolutely never broadcast on social media platform. Please Review and Complete Parent Contract: It is the intention of theis CONTRACT to promote fair play and respect for all the participants within the Southern IceDog Hockey Club. All parents must agree to this pledge before being allowed to participate in hockey and mist continue to observe the principles of Fair Play. Fair Play Code: I will not force my child to participate in hockey. I will remember that my child plays hockey for his or her enjoyment, not mine. I will encourage my child to play by the rules and to resolve conflict without resorting to hostility or violence. I will make my child feel like a winner everytime by offering praise for competing fairly and playing my best. I will never ridicule or yell at any participant for making a mistake or losing a game. I will remember that children and youth learn by example. I will applaud good plays and performances by both my child’s team and their opponents. I will never question the official’s judgment or honesty in public. I recognize officials are developing in the same manner as players, I will support all efforts to remove verbal and physical abuse from hockey games.
